TO STUDENTS WHO WANT A PHYSICAL DONE BY THE SCHOOL DOCTOR. To play a fall sport you must pick up a blank physical form in the nurses office. All forms must be completed and signed by parent. Completed forms must be dropped off at the nurse office no later than May 30, 2008. Physcials done by private physicians must be dated no earlier than June 1,2008.
Appointment cards will be sent out with Date/Time of sports physicals. You must have a completed physical form to recieve an appointment card. Submitted by Mrs. Nardell
